The health benefits of Tai Chi have been studied for decades. A remarkable new umbrella review brings together evidence from 1,744 randomised controlled trials. As a long-time Tai Chi practitioner and acupuncturist, I take a look at what the research actually tells us.
I have practised Tai Chi for many years. Like many people who have stayed with it for a long time, I have experienced its benefits not simply as a form of exercise, but as a particular way of moving, breathing, concentrating and becoming aware of the body.
For a long time, however, there has been an interesting gap between the experience of practitioners and the scientific evidence. There have been many studies of Tai Chi, and numerous systematic reviews and meta-analyses, but the findings have sometimes been scattered or inconsistent.
That makes a major new review published in the British Journal of Sports Medicine particularly interesting.

Researchers have brought together 92 systematic reviews and meta-analyses containing 1,744 randomised controlled trials investigating traditional Chinese exercises in people with musculoskeletal disorders.
The review found evidence for benefits in several important areas, including physical function, pain, stiffness, mental health, sleep and bone mineral density. The evidence was not equally strong for every outcome, but some findings reached high certainty.
And Tai Chi was the most extensively studied of the traditional Chinese exercises included.
What exactly did the researchers study?
The paper is an umbrella review.
An ordinary systematic review brings together the results of individual studies. A meta-analysis may then statistically combine their results.
An umbrella review goes another level up: it systematically examines existing systematic reviews and meta-analyses.
That makes this type of research particularly useful when a field has accumulated a large amount of evidence and we want to ask a broader question:
What does the whole body of research actually tell us?
In this case, the researchers examined evidence concerning traditional Chinese exercises including Tai Chi, Qigong, Baduanjin, Wuqinxi and Yijinjing.
Across the 92 evidence syntheses were 1,744 randomised controlled trials.
The review examined 547 different associations between these exercises and health outcomes. Of these, 23 were judged to have high-certainty evidence and 179 moderate-certainty evidence. Around 71% of the included meta-analyses were considered to be of high methodological quality.
That is a substantial body of evidence.
It is also worth stressing that this is not evidence that Tai Chi is a cure for every illness. The review specifically concerned musculoskeletal disorders, and the strength of the evidence varied considerably between outcomes.
That distinction matters.
What did the researchers find?
The results were particularly interesting in several areas.
Tai Chi and chronic musculoskeletal pain
One of the most relevant findings was evidence that Tai Chi can reduce chronic musculoskeletal pain and stiffness.
This is perhaps unsurprising to someone who has practised Tai Chi for a long time. The movements are generally slow and controlled, but they are not simply passive stretching.
The practitioner is continually shifting weight, coordinating different parts of the body, maintaining balance and controlling movement.
The research suggests that this combination can have clinically meaningful effects for people suffering from chronic musculoskeletal problems.
Tai Chi should not, of course, be regarded as a replacement for appropriate medical assessment or treatment. But the evidence increasingly supports it as one useful component of managing chronic musculoskeletal conditions.
Tai Chi and osteoarthritis
Osteoarthritis is one of the areas in which Tai Chi has been studied particularly extensively.
The new review found high-certainty evidence for improvements in physical function in people with knee osteoarthritis when traditional Chinese exercises were considered as a group.
There is a practical reason why Tai Chi can be particularly attractive in this context.
People with painful joints are sometimes caught in a difficult cycle: movement hurts, so they move less; reduced activity then contributes to weakness, stiffness and loss of confidence; and movement subsequently becomes even more difficult.
Tai Chi provides a relatively gentle way of practising movement, weight transfer, balance and coordination.
The aim isn’t to force a painful joint through a large range of movement. Instead, the practitioner gradually becomes more capable of moving with control and awareness.
The research suggests that sustained practice can translate into meaningful improvements in physical function.
Tai Chi and osteoporosis
Perhaps one of the most striking findings concerns bone mineral density.
The review found evidence that Tai Chi can increase bone mineral density in people with osteopenia or osteoporosis.
This is particularly interesting because osteoporosis presents a somewhat different challenge from many other musculoskeletal conditions.
With osteoporosis, maintaining appropriate physical activity is important, but exercise has to be selected with care, particularly where there is a significant risk of falling or fracture.
Tai Chi combines movement with continual balance training and weight shifting. It therefore has potential benefits beyond simply exercising individual muscles.
The review also found that, for some outcomes involving osteoporosis and knee osteoarthritis, greater benefits were associated with longer programmes, greater frequency and greater weekly exercise volume.
This is an important point.
The evidence is not really saying that doing five minutes of Tai Chi occasionally will transform your health.
As with most forms of exercise, consistency appears to matter.

Tai Chi, balance and physical function
One of the things that can be difficult to appreciate if you have never practised Tai Chi is how much physical work is involved in apparently slow movement.
A Tai Chi form may look effortless from the outside.
Inside the body, however, there is a continual process of:
- shifting weight from one leg to the other
- controlling the position of the pelvis and spine
- maintaining balance
- coordinating the arms and legs
- regulating the speed and direction of movement
- adjusting muscular tension
- maintaining attention
This makes Tai Chi particularly interesting for older adults.
Maintaining the ability to move confidently and safely is enormously important as we age. Loss of strength and balance can lead to reduced activity, falls and loss of independence.
Tai Chi isn’t the only form of exercise that can address these problems, and the evidence should not be interpreted as showing that it is necessarily superior to every other form of exercise.
But it is a remarkably accessible form of movement for many people.
Tai Chi and mental health
The new review also found high-certainty evidence for improvements in mental health associated with traditional Chinese exercises in people with knee osteoarthritis and neck pain.
This is another area where the distinction between Tai Chi and conventional exercise becomes interesting.
Tai Chi is physical exercise, but it is not only physical exercise.
Attention is directed towards movement. Breathing is coordinated with the activity. The practitioner is encouraged to become aware of posture, balance and bodily sensation.
That combination of physical movement and sustained attention is one reason Tai Chi is often described as a mind-body practice.
The research does not mean that Tai Chi should replace appropriate psychological or psychiatric treatment.
But it does suggest that the mental and physical effects of the practice may be connected rather than completely separate.
Tai Chi and sleep
Sleep is another area in which the review found encouraging evidence.
High-certainty evidence supported improvements in sleep quality in people with fibromyalgia when traditional Chinese exercises were considered.
This is particularly relevant because chronic pain and poor sleep can reinforce one another.
Pain can interfere with sleep, poor sleep can make pain feel more difficult to manage, and reduced activity can then contribute to further physical deterioration.
An intervention that can potentially influence several parts of this cycle at once is therefore particularly interesting.
How much Tai Chi do you need to do?
One of the most useful aspects of the review is that the researchers didn’t only ask whether traditional Chinese exercise worked. They also examined whether characteristics such as duration, frequency and weekly exercise volume influenced the results.
For some outcomes, particularly knee osteoarthritis and osteoporosis, greater benefits were associated with longer and more frequent practice.
The strongest results tended to occur with programmes lasting six months or more, practised at least three times per week, with around 150 minutes of exercise per week.
This shouldn’t be interpreted as a magic prescription. Tai Chi studies use different styles, forms, teaching methods and durations, and the optimal amount of practice will inevitably vary between individuals.
But it does reinforce something that experienced practitioners already know:
Tai Chi is a practice, not a pill.
Its effects are likely to depend to some extent on actually practising it regularly.
Why might Tai Chi work?
Modern research can tell us that an intervention produces a particular effect without necessarily telling us exactly why.
There are probably several mechanisms involved in Tai Chi.
At a straightforward physiological level, it provides physical activity. It challenges balance and coordination, uses the muscles, requires controlled weight transfer and can improve physical function.
There may also be effects associated with breathing, relaxation, attention and stress regulation.
And there is another possibility which is particularly interesting from the perspective of Chinese medicine.
Traditional Chinese medicine does not generally divide the person into completely separate systems in the way modern medicine often does. Movement, breathing, emotional state, circulation, digestion and general vitality are understood as interconnected.
Tai Chi grew out of this same traditional culture.
That doesn’t mean that modern scientific research has demonstrated the existence of every traditional Chinese medical concept.
It does mean that some of the things Tai Chi practitioners have traditionally valued — coordinated movement, relaxed breathing, awareness and regular practice — are now being investigated using completely different scientific methods.
I find that meeting between the two perspectives fascinating.
Tai Chi and acupuncture
As an acupuncturist, I am particularly interested in the relationship between Tai Chi and acupuncture.
They are obviously not the same thing.
Acupuncture is a treatment involving the insertion of needles at specific points. Tai Chi is a form of movement practice.
But both developed within the broader tradition of Chinese medicine and both place considerable emphasis on the quality and regulation of movement, bodily awareness and the relationship between different aspects of the person.
There is also an important practical distinction.
Acupuncture is something that a practitioner does to you.
Tai Chi is something you learn to do yourself.
That makes it potentially valuable as a form of self-care.
A person may come for acupuncture because they are experiencing pain or stiffness. But helping someone become more comfortable and confident in moving their own body can be an equally important part of a long-term approach to health.
For some people, Tai Chi may therefore sit very naturally alongside acupuncture and other forms of treatment.
What the new research does — and doesn’t — prove
It is tempting, when confronted with a study involving 1,744 randomised controlled trials, to conclude that the question has been settled completely.
Science rarely works quite like that.
The umbrella review provides unusually strong evidence for some outcomes, but not every finding was supported by high-certainty evidence.
The studies also involved different forms of traditional Chinese exercise, different populations, different conditions and different treatment protocols.
Tai Chi itself is not a single standardised exercise in the way that taking a particular drug is.
There are different styles and forms, different teaching methods and considerable variation in how people practise.
So we should be careful about making claims such as “Tai Chi prevents osteoporosis” or “Tai Chi treats arthritis”.
The evidence is much more interesting than that.
It suggests that regular Tai Chi can be a useful therapeutic exercise for people with a range of musculoskeletal problems, with particularly encouraging evidence concerning pain, stiffness, physical function and bone mineral density.
And it suggests that the benefits may become more apparent with sustained practice.
What I find most interesting about the research
After practising Tai Chi for many years, I don’t find it particularly surprising that researchers are finding benefits from it.
What is remarkable is the scale at which the subject is now being studied.
There was a time when Tai Chi could easily be dismissed in Western medicine as an interesting traditional practice for which there was little scientific evidence.
That position is becoming increasingly difficult to maintain.
We now have thousands of randomised controlled trials, systematic reviews and meta-analyses investigating different aspects of traditional Chinese exercise.
The new umbrella review doesn’t mean that every traditional claim about Tai Chi has been scientifically validated.
But it does mean that we can make some claims with considerably greater confidence than we could in the past.
And perhaps that is the most interesting development of all.
Tai Chi doesn’t have to choose between being a traditional Chinese practice and being investigated scientifically.
It can be both.
A practice rather than a quick fix
There is one final point that I think is easy to lose in all the discussion of studies and statistics.
Tai Chi is something you practise.
Its value isn’t necessarily found in one dramatic workout. It is found in repeating relatively simple movements until they become increasingly familiar, coordinated and comfortable.
That is one of the things I have always found appealing about it.
You don’t have to be young, athletic or particularly flexible to begin.
You don’t have to perform the movements perfectly.
And you don’t necessarily need a large amount of equipment or an expensive gym membership.
You simply have to begin moving.
The new research provides strong reasons for taking that practice seriously.
For people living with chronic musculoskeletal pain, stiffness, osteoarthritis, osteoporosis or difficulties with physical function, Tai Chi is no longer merely something that can be recommended because it “seems to do people good”.
There is now a substantial scientific literature behind it.
And for those of us who have been practising Tai Chi for many years, it is gratifying to see something that has long been experienced from the inside being examined so carefully from the outside.
